Caregiver Feedback/Survey

The caregiver is expected to establish and maintain professional communication between the Contractor’s Program Director and the Behavioral Aide assigned to the High-Risk Youth under the caregiver’s care and direction. The caregiver is requested to communicate formally monthly with the CPD and/or their designee, Compliance Monitor to allow Contractor to obtain caregiver satisfaction information. Caregiver satisfaction is an important external indicator of the success and effectiveness of the services provided and can be measured through caregiver complaints.

Caregivers will be provided with the 24/7 Complaint Hotline (1-800- 913-3530) and a web-based form that they can complete providing information that can allow Contractor to investigate the issues and/or problems. The caregiver can also always complain to DCFS staff and DCFS staff can convey the concerns to Contractor.
